Plan Comparison

Consumer Directed Choices offers two plans for dental insurance from which you can pick:

  • Delta Dental PPO
  • DeltaCare USA

Delta Dental PPO

If you select Delta Dental PPO, you receive the greatest benefit if you go to a Dentist within the network. Those Dentists in the network have contracted with Delta Dental to provide services at a reduced rate. If you decide to use a Dentist outside of the network, you pay the coinsurance at the same percentage rate, however, the base fee the Dentist will charge will be more, therefore your out of pocket costs will be more.

This plan features:

  • An annual deductible of $50.00 per person or $150.00 per family
  • A coinsurance amount that varies by procedure
  • A benefit maximum of $2000.00 per calendar year

DeltaCare USA

If you select DeltaCare USA, you must go to a Dentist in the contracted HMO network and you must select that Dentist at the time you sign up for benefits. Please note that just because a Dentist “accepts” Delta Dental insurance does not mean that he or she is a contracted network provider. Be sure to ask the Dentist if he/she is “a contracted network provider with DeltaCare USA”. You can also find out if a Dentist is a contracted network provider by going on Delta Dental’s Website and go to “Find a Dentist” on the right side of the screen. Select your dental plan and click “Search.”

This plan features:

  • No deductibles
  • No coinsurance
  • No annual or lifetime benefit maximums
  • A predefined amount you pay for each service
